From the moment you call Elder Services, we begin considering the specific needs of your loved ones. On the initial call, we ask questions to form a basic needs assessment. Then we set up an in-home visit with the family – including the family member who may ultimately benefit from our care. This visit allows us to gather even more information, answer questions and let your loved ones get a feel for how Elder Services could help them and their family. With Elder Services, you will always know what services are being provided to your loved one.

The Process

Step One

The first step in our process includes a personal consultation with an Elder Services coordinator. One of our care coordinators will be happy to meet with you, discuss your needs, and assist with a personalized care plan specific to the level of care you need.

Step Two

After our initial meeting we develop a Custom Care Plan for your loved one. This plan is tailored to a person's specific needs, and we make detailed recommendations on exactly what kind of services we feel would benefit them.

Step Three

We then arrange a follow-up meeting to go over the Custom Care Plan in detail and modify it to meet your expectations. After you are satisfied, we can implement the Custom Care Plan and begin to give your loved ones the care they expect and deserve.

Our home care services are available around the clock, every day of the year and they include everything from companionship, meal preparation, light housekeeping, medication reminders, shopping and errands to continual care for Alzheimer's and Dementia care.
